Overview

The water and dialysate quality condition at 42 CFR Part 494.40 is the most technically specific condition in the ESRD Conditions for Coverage — requiring that dialysis facilities produce water and dialysate that meet the chemical and microbiological standards established by the Association for the Advancement of Medical Instrumentation (AAMI), as referenced in the CMS interpretive guidance. These standards specify maximum allowable concentrations for a long list of chemical contaminants, maximum allowable bacterial counts, and maximum allowable levels of endotoxin — all of which must be verified through systematic testing and documented in detailed water treatment logs.

Water treatment failures in dialysis settings are not abstract regulatory violations. They are patient safety events. Contaminated dialysis water — whether chemically contaminated by treatment chemicals, biologically contaminated by bacteria or endotoxin, or physically compromised by equipment failure — can cause acute hemolytic reactions, systemic infections, and death in patients who are already clinically vulnerable due to their ESRD. Water Treatment System Configuration Assessment

We assess the configuration of your water treatment system — including the sequence and adequacy of treatment components (softeners, carbon tanks, reverse osmosis units, deionization systems, and ultrafilters) — against AAMI standards for water treatment system design. Systems that are inadequately configured for the quality of the municipal or well water supply being treated are a fundamental compliance risk that policies and testing protocols alone cannot address.

Chemical Testing Protocol Compliance

The Conditions for Coverage require that dialysis water be tested for a specific list of chemical contaminants at defined frequencies — with testing performed by qualified personnel using appropriate testing methods and documentation maintained in water treatment logs.
